WIAA And Youth Football Physicals Tuesday

Memorial Medical Center will again be offering WIAA and Youth Football Physicals for the 2013/ 2014 year on Tuesday (8/6) in Neillsville.Physicals will be held:
Tuesday, August 6th in Neillsville (3:00p.m. - 6:00 p.m.)
? An appointment is required.
? A fee of $20.00 is due the day of the physical.
? Insurance will not be filed for physicals.
? Athletes must bring completed WIAA Sports Physical Forms with them on the day of their appointment (forms may be found at http://www.wiaawi.org).
? A Parent or guardian must accompany athlete to the physical appointment.
