Concussion ImPact Evaluation
Monday, July 8th, 2013 -- 9:00 AM
All Neillsville athletes for the 2013-2014 seasons are strongly encouraged to have the ImPact evaluation completed.The evaluations will take place in the high school computer lab.
The evaluation is good for two years, so if you took it last year you do not need to repeat the evaluation for this year.

The forms may be picked up in the middle/high school office. Both athlete and parent/guardian need to sign the consent form. This form needs to be brought with you when you come to the evaluation.
