Healthy People Wood County Launches 2026 Community Health Assessment Survey

Healthy People Wood County is excited to announce the launch of the 2026 Community Health Assessment (CHA) Survey.

Community members who live and/or work in Wood County are invited to complete the survey to share their experiences, priorities, and concerns, helping identify local priorities and guide future health improvement efforts. Your participation is valuable and helps us understand what matters most to you and your neighbors.

“The CHA survey is one of the most important tools we have for understanding the health needs of our community,” said Kristie Egge, Supervisor of Strategic Initiatives at Wood County Health Department. “By hearing directly from residents, we can make informed decisions and develop strategies that truly reflect community priorities.”

The survey was created by the Wood County Health Department and Aspirus Wisconsin Rapids to get the community's opinion on what impacts health in Wood County.

“Working closely with the Wood County Health Department on this effort benefits the whole community. By combining resources, we are able to make an impact that we cannot do alone,” noted Sarah Beversdorf, Community Benefit Analyst at Aspirus.

All survey responses are anonymous, and the survey takes about 10 minutes to complete. After completing the survey, you’ll have the chance to enter to win one of three $50 VISA gift cards as a thank you for sharing your thoughts. 

How to Participate:

Complete the survey online in English or Spanish via the secure link: https://bit.ly/WoodCHA or by scanning the QR code.                          

 Paper copies are available and can be returned at the following locations:
