WIAA and Youth Football Physicals To Be Offered

Memorial Medical Center is proud to announce the offering of WIAA and Youth Football Physicals for the 2012 / 2013 year.
Physicals will be held on the following dates at various locations:
Monday, July 16th in Neillsville (12:00 - 7:00 p.m.)
Tuesday, July 17th in Loyal (2:30 - 7:00 p.m.)
Thursday, July 26th in Greenwood (4:00 - 8:00 p.m.)
Tuesday, August 7th in Neillsville (4:00 - 6:00 p.m.)
Appointments are required, and a fee of $20.00 is due the day of the physical. Insurance will not be filed for physicals.
Athletes must bring completed WIAA Sports Physical Forms with them on the day of their appointment (forms may be found at wiaawi.org). A Parent or guardian must accompany athlete to the physical appointment.
Memorial Medical Center will donate all fees received from these physicals back to the Athletic Department of the athletes. Call 715.743.3101 for appointments and mention Sports Physical Special when making your appointment.
